A healthcare organization New York City is currently seeking an experienced, licensed Registered Nurse (RN) to join their team as a Care Manager. The role is hybrid during training and fully remote after training.
About the Opportunity:
- Setting: Managed Care (MLTC)
- Patient Population: Adults & Geriatrics
Responsibilities:
The Care Manager (RN) will:
- Provide case management to members, including medical, social, psychological, and physical.
- Develop, implement, and monitor the care plan to help meet the member's needs
- Perform other duties, as needed
Qualifications:
- 2+ years of Case Management and/or Home Care experience
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
Desired Skills:
- Bachelor's Degree in Nursing
- Bilingual (English and Cantonese, Mandarin, or Spanish)
