Responsibilities
As a Nurse Practitioner, you will conduct annual risk assessments in a residential setting or via telehealth, in accordance with your scope of practice and under the supervision of a Collaborative Physician.
Essential Duties and Responsibilities
- Conduct comprehensive in-home health assessments to identify active and chronic conditions and evaluate physical, mental, and social needs during each visit.
- Perform patient history-taking, examinations, and diagnoses.
- Document visits in writing per NCQA standards.
- Take vital signs as necessary.
- Facilitate case management referrals and communicate with primary care providers as required.
- Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnoses.
- Adhere to OSHA Universal Precautions while performing clinical duties.
- Maintain patient confidentiality at all times.
- Attend required meetings and in-services and participate in committees as requested.
- Engage in professional development activities to maintain licenses and professional affiliations.

Qualifications
Required Knowledge, Skills, and Experience
- Active/unrestricted nurse practitioner license in appropriate states.
- Board certification from organizations such as ANCC, AANP, or NCCPA.
- Current CPR Certification.
- Enrollment in Medicare/Medicaid.
- Valid driver’s license with a good driving record.
- Proficient in EHR systems.
Preferred Knowledge, Skills, and Experience
- Experience or training in geriatrics.
- Teamwork skills and the ability to maintain effective relationships with patients, medical staff, and the public.

- Ability to adapt to varying weather conditions and home environments.
- Full range of motion to perform tasks, including lifting and handling patients.
- Regular lifting of items weighing up to 50 pounds.
- Capacity to travel up to 150 miles daily in urban or rural settings by vehicle.
Compensation
Posted Min Pay Rate: USD $85.00/Visit
Posted Max Pay Rate: USD $145.00/Visit
