Responsibilities

The Nurse Practitioner performs annual risk assessments in residential settings or via telehealth, following the scope of practice as delegated by the Collaborative Physician.

Essential Duties and Responsibilities

  • Conduct comprehensive in-home health risk assessments to identify existing and chronic conditions and evaluate physical, mental, and social needs during the visit.
  • Take patient history, perform examinations, and establish diagnoses.
  • Document patient visits according to NCQA standards.
  • Measure patient vital signs as necessary.
  • Initiate case management referrals and communicate with the primary care physician as needed.
  • Engage with patients, caregivers, agency nurses, other providers, and vendors to ensure accurate diagnoses and effective care.
  • Practice all clinical duties under OSHA Universal Precautions.
  • Maintain confidentiality of patient information.
  • Attend required meetings and training sessions, and participate in committees as requested.
  • Engage in professional development activities and uphold professional licenses and affiliations.

Qualifications

Required Knowledge, Skills, and Experience

  • Active/unrestricted nurse practitioner license to practice in relevant states.
  • Board certification from one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).
  • Current CPR Certification.
  • Enrolled in Medicare/Medicaid.
  • Valid driver's license and a good driving record.
  • Strong EHR skills.

Preferred Knowledge, Skills, and Experience

  • Experience or training in geriatric care.
  • Ability to work effectively with patients, medical staff, and the public.

Conditions of this role include:

  • Ability to adapt to various weather conditions and different patient home environments.
  • Full range of body motion capabilities, including the ability to handle and lift patients.
  • Regular lifting/carrying of items up to 50 pounds.
  • Willingness to travel up to 150 miles daily in both urban and rural settings.
  • Must be able to drive, if required.

Compensation:

Minimum Pay Rate: USD $85.00/Visit

Maximum Pay Rate: USD $145.00/Visit

Note: Individual compensation packages are tailored based on factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
