Responsibilities

As a Nurse Practitioner, you will conduct annual risk assessments in patients' homes or via telehealth, following the scope of practice delegated by the Collaborative Physician.

Essential Duties and Responsibilities

  • Perform in-home health risk assessments, identifying active and chronic diseases, as well as physical, mental, and social needs during the visit.
  • Take patient history, conduct examinations, and establish diagnoses.
  • Provide thorough documentation of patient visits in accordance with NCQA standards.
  • Monitor and record patient vital signs as necessary.
  • Facilitate case management referrals and maintain communication with primary care providers.
  • Engage with patients, caregivers, agency nurses, and other healthcare providers to ensure accurate diagnoses and care plans.
  • Adhere to OSHA Universal Precautions while performing clinical duties.
  • Maintain patient confidentiality and actively participate in team meetings and professional development activities.

Qualifications

Required Knowledge, Skills, and Experience

  • Active and unrestricted medical license.
  • Current CPR certification.
  • Board eligible or board-certified in family medicine.
  • Proficiency in electronic health records (EHR).

Preferred Knowledge, Skills and Experience

  • Experience or training in geriatric care.
  • Strong teamwork skills and the ability to build effective relationships with patients, medical staff, and the community.

Conditions to be Aware of

  • Ability to adapt to varying weather and residential conditions.
  • Capacity for comprehensive physical activities including lifting and handling patients.
  • Regular lifting/carrying of items up to 50 pounds.
  • Willingness to travel up to 150 miles daily in urban and rural settings; possessing a valid driver's license may be necessary.
