Responsibilities
As a Nurse Practitioner, you will conduct annual risk assessments in patients' homes or via telehealth, operating within your scope of practice under the guidance of a Collaborative Physician.
Essential Duties and Responsibilities
- Perform comprehensive in-home health risk assessments to identify all active and chronic conditions, and assess individual physical, mental, and social needs.
- Gather patient histories, conduct examinations, and determine diagnoses.
- Document patient visits in accordance with NCQA standards.
- Monitor patient vital signs as needed.
- Coordinate case management referrals and communicate with primary care providers.
- Engage in effective communication with patients, caregivers, agency nurses, and other healthcare professionals to ensure accurate diagnoses.
- Execute clinical responsibilities while adhering to OSHA Universal Precautions.
- Maintain patient confidentiality at all times.
- Attend required meetings and training sessions, and participate in committees as needed.
- Engage in professional development activities to maintain licenses and affiliations.

Qualifications
Required Knowledge, Skills, and Experience
- Active, unrestricted nurse practitioner license in the coverage states.
- Board certification from ANCC, AANP, or NCCPA.
- Current CPR certification.
- Enrollment in Medicare/Medicaid programs.
- Valid driver's license and good driving record.
- Strong EHR skills.
Preferred Knowledge, Skills, and Experience
- Experience or training in geriatric care.
- Ability to work effectively in a team and maintain strong relationships with patients and staff.
Conditions of this role:
- Ability to adapt to various weather conditions and residential environments.
- Physical capability to handle and lift patients, including full range of body motion.
- Capable of regular lifting/carrying items up to 50 pounds.
- Ability to travel in an automobile or van for up to 150 miles daily.
Pay Rates
Min pay rate: USD $85.00/Visit
Max pay rate: USD $145.00/Visit
