Responsibilities

As a Nurse Practitioner, you will conduct annual risk assessments either in a residential setting or via telehealth within your scope of practice as delegated by the Collaborative Physician.

Essential Duties and Responsibilities

  • Perform comprehensive in-home health risk assessments to identify active and chronic health conditions, as well as mental and social needs.
  • Take medical histories, conduct examinations, and determine diagnoses.
  • Provide thorough written documentation for each patient visit in accordance with NCQA standards.
  • Measure and record patient vital signs as necessary.
  • Make case management referrals and communicate with primary care providers when needed.
  • Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnoses.
  • Execute all clinical duties while maintaining adherence to OSHA Universal Precautions.
  • Uphold patient confidentiality at all times.
  • Participate in required meetings, in-services, and committees as directed.
  • Engage in professional development activities to maintain licenses and affiliations.

Qualifications

Required Knowledge, Skills, and Experience

  • Active and unrestricted Nurse Practitioner license in the relevant states
  • Board certification from ANCC, AANP, or NCCPA
  • Current CPR certification
  • Enrollment in Medicare/Medicaid
  • A valid driver's license and a good driving record
  • Proficient EHR skills

Preferred Knowledge, Skills, and Experience

  • Experience or training in geriatric care
  • Strong interpersonal skills and ability to foster effective working relationships

Conditions of this role to be aware of:

  • Ability to adapt to various weather conditions and home environments of patients
  • Physical capability for lifting patients and performing tasks requiring dexterity and coordination
  • Regularly lift/carry items weighing up to 50 pounds
  • Willingness to travel up to 150 miles daily in urban or rural settings

Compensation:

Posted Min Pay Rate: USD $85.00/Visit

Posted Max Pay Rate: USD $145.00/Visit
