Responsibilities
As a Nurse Practitioner, you will conduct annual risk assessments in the comfort of patients' homes or via telehealth, following the guidelines provided by the Collaborative Physician.
Key Duties Include:
- Perform comprehensive in-home health risk assessments to identify patient conditions and needs.
- Conduct patient histories, examinations, and diagnoses.
- Document patient visits according to NCQA standards.
- Take vital signs as necessary.
- Make case management referrals and consult with primary care physicians as needed.
- Maintain effective communication with patients, caregivers, nurses, and providers.
- Adhere to OSHA Universal Precautions while performing clinical duties.
- Ensure patient confidentiality at all times.
- Participate in required meetings and training sessions.
- Engage in professional development activities to sustain your licenses and affiliations.

Qualifications
Required:
- Active/unrestricted nurse practitioner license.
- Board certification from ANCC, AANP, or NCCPA.
- Current CPR certification.
- Enrollment in Medicare/Medicaid programs.
- Valid driver's license with a clean driving record.
- Proficient in EHR systems.
Preferred:
- Experience with geriatric patients.
- Excellent interpersonal skills and ability to build relationships.
Conditions of the Role:
- Ability to adapt to varying weather and home environments.
- Physical ability to handle/lift patients and perform necessary movements.
- Ability to lift/carry items weighing up to 50 pounds.
- Willingness to travel up to 150 miles daily, driving when necessary.
Compensation:
Min Pay Rate: USD $85.00/Visit
Max Pay Rate: USD $145.00/Visit
Pay Transparency: Compensation packages vary based on individual candidate qualifications and experience.
