Responsibilities

The Nurse Practitioner will conduct annual risk assessments either in-person at residences or via telehealth, in accordance with their scope of practice and under the guidance of a Collaborative Physician.

Essential Duties and Responsibilities

  • Conduct thorough in-home health risk assessments to identify both active and chronic disease conditions as well as assess physical, mental, and social needs.
  • Gather patient history, perform examinations, and establish diagnoses.
  • Document patient visits in writing, adhering to NCQA standards.
  • Monitor vital signs as necessary.
  • Facilitate case management referrals and communicate with Primary Care Physicians as needed.
  • Engage with patients, caregivers, agency nurses, and other healthcare providers to ensure accurate diagnosis and care.
  • Maintain compliance with OSHA Universal Precautions and ensure patient confidentiality.
  • Participate in required meetings, in-services, and committees.
  • Engage in professional development and maintain required licenses and affiliations.

Qualifications

Required Knowledge, Skills, and Experience

  • Active/unrestricted medical license.
  • Current CPR Certification.
  • Board eligibility or certification in family medicine.
  • Strong EHR proficiency.

Preferred Knowledge, Skills, and Experience

  • Experience or training in geriatrics.
  • Ability to work collaboratively and maintain strong relationships with patients and medical staff.

Conditions of Employment

  • Capacity to adapt to varying weather conditions and different home environments.
  • Ability to perform a full range of physical activities, including lifting patients, manual dexterity, and mobility.
  • Regularly lift/carry items weighing up to 50 pounds.
  • Willingness to travel up to 150 miles daily in urban and/or rural settings.
