ResponsibilitiesThe Nurse Practitioner will conduct annual risk assessments either in-person at residences or via telehealth, in accordance with their scope of practice and under the guidance of a Collaborative Physician.
Essential Duties and Responsibilities
- Conduct thorough in-home health risk assessments to identify both active and chronic disease conditions as well as assess physical, mental, and social needs.
- Gather patient history, perform examinations, and establish diagnoses.
- Document patient visits in writing, adhering to NCQA standards.
- Monitor vital signs as necessary.
- Facilitate case management referrals and communicate with Primary Care Physicians as needed.
- Engage with patients, caregivers, agency nurses, and other healthcare providers to ensure accurate diagnosis and care.
- Maintain compliance with OSHA Universal Precautions and ensure patient confidentiality.
- Participate in required meetings, in-services, and committees.
- Engage in professional development and maintain required licenses and affiliations.

QualificationsRequired Knowledge, Skills, and Experience
- Active/unrestricted medical license.
- Current CPR Certification.
- Board eligibility or certification in family medicine.
- Strong EHR proficiency.
Preferred Knowledge, Skills, and Experience
- Experience or training in geriatrics.
- Ability to work collaboratively and maintain strong relationships with patients and medical staff.
Conditions of Employment
- Capacity to adapt to varying weather conditions and different home environments.
- Ability to perform a full range of physical activities, including lifting patients, manual dexterity, and mobility.
- Regularly lift/carry items weighing up to 50 pounds.
- Willingness to travel up to 150 miles daily in urban and/or rural settings.
