Responsibilities

The Nurse Practitioner conducts comprehensive annual risk assessments in patients' homes or through telehealth, practicing within their scope as delegated by a Collaborative Physician.

Key Responsibilities:

  • Conduct thorough in-home health risk assessments to identify active and chronic disease conditions, as well as assessing physical, mental, and social needs.
  • Gather patient history, perform examinations, and establish diagnoses.
  • Document patient visits adhering to NCQA standards.
  • Measure vital signs as needed.
  • Make case management referrals and collaborate with Primary Care Providers as necessary.
  • Communicate clearly with patients, caregivers, agency nurses, and other providers to ensure accurate diagnoses.
  • Perform all clinical duties while observing OSHA Universal Precautions.
  • Maintain confidentiality of patient information.
  • Attend required meetings and trainings and participate in committees as requested.
  • Engage in professional development activities, maintaining licenses and affiliations.

Collaboration:

  • Work closely with teammates, physicians, medical staff, patients, caregivers, agency nurses, and vendors.

Qualifications

Required Skills and Experience:

  • Active/unrestricted medical license.
  • Current CPR Certification.
  • Board eligible or board certified in family medicine.
  • Proficient in electronic health records (EHR).

Preferred Experience:

  • Experience or training in geriatric care.
  • Ability to work effectively in a team-oriented environment and foster good relationships with patients and staff.

Conditions to Consider:

  • Ability to adapt to various weather conditions and residential environments.
  • Must have full range of motion, capable of handling/lifting patients, with manual dexterity and normal visual/auditory capabilities.
  • Regularly lift/carry items weighing up to 50 pounds.
  • Willingness to travel up to 150 miles daily in urban or rural settings. Ability to drive when necessary.
