Responsibilities
As a Nurse Practitioner, you will conduct annual risk assessments in-person or through telehealth, within the scope of your practice as delegated by the Collaborative Physician.
Key Responsibilities
- Conduct thorough in-home health risk assessments to identify active and chronic disease conditions, along with physical, mental, and social needs.
- Take patient histories, perform examinations, and establish diagnoses.
- Document patient visits according to NCQA standards.
- Monitor vital signs as necessary.
- Coordinate case management referrals and communicate with the patient's primary care physician as required.
- Engage with patients, caregivers, agency nurses, other providers, and vendors to ensure accurate diagnoses.
- Uphold OSHA Universal Precautions during all clinical activities.
- Maintain patient confidentiality.
- Participate in required meetings and committees as requested.
- Engage in professional development activities to maintain licenses and affiliations.

Qualifications
Required Skills and Experience
- Active/unrestricted Nurse Practitioner license in coverage states.
- Board certification from ANCC, AANP, or NCCPA.
- Active CPR Certification.
- Current enrollment in Medicare/Medicaid.
- Valid driver's license and clean driving record.
- Exceptional EHR skills.
Preferred Skills and Experience
- Experience or training in geriatrics.
- Strong teamwork skills and the ability to maintain effective relationships with patients, medical staff, and the public.
Additional Conditions
- Ability to adapt to various weather conditions and patient home environments.
- Full range of motion, including lifting and moving patients. Manual and finger dexterity, normal visual and auditory acuity, standing, bending, walking, and climbing stairs.
- Regular lifting/carrying items weighing up to 50 pounds.
- Ability to drive up to 150 miles daily in both urban and rural settings.
Compensation
Starting pay is $85.00 per visit, with a maximum of $145.00 per visit. Compensation packages are tailored based on individual experience, qualifications, and other job-related factors.
