Responsibilities
As a Nurse Practitioner, you will conduct annual risk assessments either in-person at patients' homes or through telehealth sessions, within your practice scope as delegated by the Collaborative Physician.
Key Duties and Responsibilities
- Perform comprehensive in-home health risk assessments identifying active and chronic health conditions, in addition to physical, mental, and social needs at the time of the visit.
- Gather patient history, conduct examinations, and establish diagnoses.
- Document patient visits accurately per NCQA standards.
- Measure and record patient vital signs when necessary.
- Coordinate case management referrals and communicate with the Primary Care Provider (PCP) as needed.
- Engage with patients, their caregivers, agency nurses, other medical providers, and vendors to ensure accurate diagnoses.
- Execute all clinical duties while adhering to OSHA Universal Precautions.
- Maintain the confidentiality of patient information.
- Participate in required meetings, in-services, and committees as requested.
- Pursue professional development activities to maintain licenses and professional affiliations.

Qualifications
Required Knowledge, Skills, and Experience
- Active/unrestricted Nurse Practitioner license to practice.
- Board certification from ANCC, AANP, or NCCPA.
- Current CPR Certification.
- Enrollment in Medicare/Medicaid.
- Valid driver's license and a good driving record.
- Outstanding Electronic Health Record (EHR) skills.
Preferred Knowledge, Skills, and Experience
- Experience or training in geriatric care.
- Ability to work collaboratively and maintain positive relationships with patients, medical staff, and the public.
Conditions of this Role
- Ability to adapt to varying weather conditions and diverse home environments.
- Full range of motion, including lifting and handling patients, manual dexterity, normal vision, and hearing capabilities, as well as the ability to stand, bend, walk, and climb stairs.
- Regularly lift/carry items weighing up to 50 pounds.
- Ability to travel in an automobile or van up to 150 miles daily in various settings, and drive when required.
Compensation
Posted Minimum Pay Rate: USD $85.00/Visit
Posted Maximum Pay Rate: USD $145.00/Visit
