Responsibilities

As a Nurse Practitioner, you will conduct annual risk assessments in residential settings or via telehealth under the guidance of a collaborative physician.

Essential Duties and Responsibilities

  • Perform thorough in-home health risk assessments to identify existing health conditions and assess physical, mental, and social needs during patient visits.
  • Gather patient history, conduct examinations, and determine diagnoses.
  • Document patient visits in accordance with NCQA standards.
  • Measure vital signs as needed.
  • Coordinate case management referrals and liaise with primary care providers when necessary.
  • Facilitate clear communication with patients, caregivers, agency nurses, and other healthcare providers to ensure accurate diagnoses.
  • Adhere to OSHA Universal Precautions in all clinical duties.
  • Maintain patient confidentiality at all times.
  • Participate in required meetings, training sessions, and committees as requested.
  • Engage in professional development activities and uphold all necessary professional licenses and affiliations.

Qualifications

Required Knowledge, Skills, and Experience

  • Active/unrestricted Nurse Practitioner license in applicable states
  • Board certification from ANCC, AANP, or NCCPA
  • Current CPR certification
  • Enrollment in Medicare/Medicaid
  • Valid driver's license with a clean driving record
  • Excellent skills in electronic health records (EHR)

Preferred Knowledge, Skills, and Experience

  • Experience or training in geriatric care
  • Strong teamwork skills and the ability to maintain effective working relationships with patients, medical staff, and the public

Conditions of this role to be aware of:

  • Ability to adapt to varying weather conditions and different home environments
  • Full range of motion required, including lifting and handling patients
  • Ability to regularly lift and carry items weighing up to 50 pounds
  • Willingness to travel up to 150 miles daily in various settings, with the ability to drive as needed
