Responsibilities
The Nurse Practitioner is responsible for conducting annual risk assessments in residential settings or via telehealth, within the scope of practice as directed by the Collaborative Physician.
Essential Duties and Responsibilities
- Conduct comprehensive in-home health risk assessments, identifying chronic and active disease conditions.
- Take patient history, perform examinations, and establish diagnoses.
- Document patient visits in accordance with NCQA standards.
- Obtain vital signs as necessary.
- Initiate case management referrals and communicate with Primary Care Providers (PCP) as needed.
- Maintain communication with patients, caregivers, agency nurses, and other providers to ensure appropriate care.
- Adhere to OSHA Universal Precautions during clinical duties.
- Safeguard patient confidentiality.
- Participate in required meetings and professional development activities.

Qualifications
Required Knowledge, Skills, and Experience
- Active, unrestricted nurse practitioner license in relevant states.
- Board certification from ANCC, AANP, or NCCPA.
- Current CPR certification.
- Enrollment in Medicare/Medicaid programs.
- A valid driver's license and a clean driving record.
- Strong skills in electronic health records (EHR).
Preferred Knowledge, Skills, and Experience
- Experience or training in geriatric care.
- Ability to foster teamwork and maintain effective relationships with diverse stakeholders.
Conditions of this role to be aware of:
- Ability to adapt to varying weather conditions and different home environments.
- Full range of motion, including handling/lifting patients, manual and finger dexterity.
- Regular lifting/carrying items up to 50 pounds.
- Capability to travel in a vehicle up to 150 miles daily, in both urban and rural settings.
Compensation
Posted Min Pay Rate: USD $85.00/Visit
Posted Max Pay Rate: USD $145.00/Visit
