Responsibilities

As a Nurse Practitioner, you will conduct annual risk assessments in a residential setting or via telehealth, working within the scope delegated by the Collaborative Physician.

Key Responsibilities:

  • Perform comprehensive in-home health risk assessments to identify active and chronic health conditions, as well as physical, mental, and social needs at each visit.
  • Take patient history, conduct examinations, and determine diagnoses.
  • Provide written documentation of patient visits, adhering to NCQA standards.
  • Record vital signs as necessary.
  • Initiate case management referrals and communicate with the patient's Primary Care Provider (PCP) as needed.
  • Ensure effective communication with patients, caregivers, agency nurses, and other healthcare providers to facilitate accurate diagnosis.
  • Adhere to OSHA Universal Precautions while performing clinical duties.
  • Maintain patient confidentiality at all times.
  • Participate in required meetings, in-services, and committees as requested.
  • Engage in professional development activities to uphold licenses and affiliations.

Qualifications

Required Skills and Experience:

  • Active, unrestricted Nurse Practitioner license to practice.
  • Board certification from ANCC, AANP, or NCCPA.
  • Current CPR certification.
  • Active enrollment in Medicare and Medicaid.
  • Valid driver's license and a good driving record.
  • Proficient in Electronic Health Records (EHR).

Preferred Skills and Experience:

  • Experience with geriatric patients.
  • Excellent teamwork skills and ability to build relationships with patients and staff.

Physical Requirements:

Please be aware of the following conditions:

  • Adaptability to varying weather conditions and diverse home environments.
  • Full range of body motion, including the lifting of patients and manual dexterity.
  • Regular lifting and carrying of items up to 50 pounds.
  • Ability to travel up to 150 miles daily by automobile as necessary.

Pay Range:

Posted Min Pay Rate: USD $85.00/Visit
Posted Max Pay Rate: USD $145.00/Visit
