Responsibilities
The Nurse Practitioner provides annual health risk assessments in residential settings or via telehealth, in accordance with the scope of practice as delegated by the Collaborative Physician.
Key Responsibilities Include:
- Conduct in-depth in-home health risk assessments to evaluate active and chronic health conditions, along with identifying physical, mental, and social needs at the time of care.
- Gather patient history, perform examinations, and establish diagnoses.
- Document patient visits according to NCQA standards.
- Measure vital signs as necessary.
- Make case management referrals and coordinate with the primary care physician.
- Maintain communication with patients, caregivers, agency nurses, other providers, and vendors to ensure accurate diagnosis and care.
- Perform clinical tasks while adhering to OSHA Universal Precautions.
- Ensure patient confidentiality is upheld at all times.
- Attend mandatory meetings and in-services, and contribute to committees as requested.
- Engage in professional development activities, ensuring ongoing licensure and professional affiliations.
Collaboration:
- Work alongside teammates, physicians, medical staff, and patients.
- Engage with caregivers, agency nurses, providers, and vendors.
Qualifications
Required Qualifications:
- Active, unrestricted nurse practitioner license in practicing states.
- Board certification through ANCC, AANP, or NCCPA.
- Current CPR certification.
- Enrollment in Medicare/Medicaid.
- Valid driver's license and a clean driving record.
- Strong EHR skills.
Preferred Qualifications:
- Experience or training in geriatric care.
- Ability to work effectively in a team and build positive relationships with patients and medical personnel.
Physical Demands:
- Ability to adapt to various weather and home environments.
- Full range of motion for handling/lifting patients; manual dexterity and coordination required.
- Ability to lift and carry items weighing up to 50 pounds regularly.
- Capacity to travel in an automobile or van up to 150 miles daily in diverse settings, with the ability to drive when necessary.
Compensation:
Posted Minimum Pay Rate: $85.00/Visit
Posted Maximum Pay Rate: $145.00/Visit
Pay Transparency: Individual compensation packages are determined by various factors unique to each candidate, including skills, experience, and qualifications.
