Responsibilities

As a Nurse Practitioner, you will conduct annual health risk assessments in the comfort of patients' homes or via telehealth, collaborating closely with our Collaborative Physician.

Essential Duties and Responsibilities

  • Perform comprehensive in-home health risk assessments to evaluate current health conditions and patient needs.
  • Conduct patient history taking, physical examinations, and diagnoses.
  • Ensure thorough documentation of visits in adherence to NCQA standards.
  • Monitor and record vital signs as needed.
  • Coordinate with case management and communicate with primary care providers as necessary.
  • Engage with patients, families, and other healthcare professionals to ensure accurate diagnoses and care plans.
  • Follow all clinical protocols while maintaining patient confidentiality.
  • Attend designated meetings, participate in in-services, and join committees as needed.
  • Pursue always professional development and maintain licensure and affiliations.

Qualifications

Required Knowledge, Skills, and Experience

  • Valid and unrestricted nurse practitioner license in relevant states.
  • Board certification from ANCC, AANP, or NCCPA.
  • Current CPR Certification.
  • Active enrollment in Medicare/Medicaid.
  • Valid driver's license with a clean driving record.
  • Strong proficiency in Electronic Health Records (EHR).

Preferred Knowledge, Skills and Experience

  • Experience in geriatric care.
  • Skilled in teamwork and fostering positive relationships within healthcare teams.

Physical Demands

  • Ability to adapt to various weather and home conditions.
  • Full range of motion required for patient handling and mobility.
  • Regularly lift/carry items weighing up to 50 pounds.
  • Willingness to travel up to 150 miles daily in various settings, with driving responsibilities as needed.

Compensation

Earn between $85.00 to $145.00 per visit based on experience and qualifications.
