Responsibilities
The Nurse Practitioner will provide annual risk assessments in a home setting or via telehealth, operating within the scope of practice as delegated by the Collaborative Physician.
Essential Duties and Responsibilities
- Conduct comprehensive in-home health risk assessments to identify active and chronic conditions and determine all physical, mental, and social needs during visits.
- Take patient histories, perform examinations, and establish diagnoses.
- Document patient visits in compliance with NCQA standards.
- Measure and record patient vital signs as needed.
- Initiate case management referrals and communicate effectively with primary care providers.
- Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnoses.
- Adhere to OSHA Universal Precautions while performing clinical duties.
- Ensure patient confidentiality is maintained at all times.
- Attend required meetings and in-services and participate in committees as needed.
- Engage in professional development and maintain necessary licenses and affiliations.

Qualifications
Required Knowledge, Skills, and Experience
- Active, unrestricted nurse practitioner license to practice.
- Board certification from ANCC, AANP, or NCCPA.
- Current CPR certification.
- Enrollment in Medicare/Medicaid programs.
- Must possess a valid driver's license and maintain a good driving record.
- Strong EHR skills are essential.
Preferred Qualifications
- Experience or training in geriatrics is preferred.
- Excellent teamwork and interpersonal skills with patients, medical staff, and the public.
Physical and Environmental Conditions
- Ability to adapt to various weather conditions and different home environments.
- Capability to handle/lift patients and perform a full range of body motions including standing, bending, walking, and stair climbing.
- Regularly lift and carry items weighing up to 50 pounds.
- Willingness to travel in a vehicle up to 150 miles daily in diverse settings.
Compensation
Hourly compensation rates range from $85.00 to $145.00 per visit based on experience and qualifications.
